INSTRUCTIONS: DEBTOR'S CERTIFICATION OF COMPLETION OF INSTRUCTIONAL COURSE CONCERNING FINANCIAL MANAGEMENT

Nearly everyone completing a Chapter 7 or 13 bankruptcy must take a course in financial management from an instructor approved by the bankruptcy court in their area. In some cases, this may also be required of Chapter 11 applicants. The Debtor's Certification of Completion of Instructional Course Concerning Financial Management must be submitted at the end of your bankruptcy process. This document can be obtained from the United States Courts system or at your local bankruptcy court. Note that while spouses can jointly file for bankruptcy, these forms must be completed and submitted separately.

Debtor's Certification of Completion of Instructional Course Concerning Financial Management Step 1: After you have completed a financial management form, list your name and case number at the top of the form where indicated.

Debtor's Certification of Completion of Instructional Course Concerning Financial Management Step 2: Where directed, print your name, the date your course was completed, and the name of the court-approved financial management provider who conducted this course.

The only people who are exempt from this requirement are people who meet the bankruptcy court's definition of being sufficiently disabled to be exempt from these proceedings, military members currently on active duty in a combat zone, or people in a district where the trustee or bankruptcy attorney handling the course have determined that no adequate course of financial management is offered.

Debtor's Certification of Completion of Instructional Course Concerning Financial Management Step 5: Sign and date the document where indicated.

Debtor's Certification of Completion of Instructional Course Concerning Financial Management Step 6: If you are filing for Chapter 7 bankruptcy proceedings, you must submit this document within 45 days of the scheduled date for the first meeting of your creditors.

Debtor's Certification of Completion of Instructional Course Concerning Financial Management Step 7: If you are filing for Chapter 11 or 13, you must submit this document by the date of your last payment as stipulated in the payment plan ordered by the court.
